An unannounced monitoring inspection was conducted to investigate a self-reported incident regarding supervision that occurred on March 15, 2022 during the operation of the unlicensed short-term program. The inspection was conducted on March 24, 2022 beginning at 11:00 AM and ending at 1:15 PM with the program director. Staff members were interviewed and documentation was reviewed. Based on the information gathered during the investigation, the self-reported incident regarding supervision was determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-F Description: Based on staff interviews, the center failed to ensure that all children under 10 years of age were within actual sight and sound supervision of staff at all times. Evidence: On March 15, 2022 at approximately 2:30 PM, Child A, 5 years of age, was left alone at Patriot Park, a community park located near Ron Rosner YMCA Recreation Center. According to information obtained through staff interviews and documentation, the child was out of staff sight and sound supervision for approximately 7 minutes. Plan of Correction: The facility will be doing refresher training for all child care staff (including transitions and missing child) as well as enforcing headcounts to be written down throughout the day.
