Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on March 1, 2022 and concluded on March 2, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 125 children present, ranging in ages from infant to five years, with 30 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. In room #3, room #4 and room #6, the bottom half of the window screens were torn.
2. In room #7, the child size sofa had tears on the bottom and arms of the sofa.
3. On the sensory playground, the wood border around the area farthest from the gate had deteriorating wood with splinters.
4. On the toddler playground, beneath the bike path material was a sunken area which created a tripping hazard.

Description: Based on observation, the facility did not ensure compliance with ratios of staff to children as required.

Evidence:
In room #2, one teacher went into the diapering and bottle storage area, closed the sliding door for appropriately one minute leaving one teacher to 6 children. The children in room #2 are 16 months or younger.
