Technical Assistance:
The following was discussed with the designated person in-charge:

8VAC20-780-(5)-350-M
One staff member shall not supervise more than one room or area during rest time.

8VAC20-780-(4)-330-B *Resilient surfacing under playground equipment
The slide on the property does not have resilient surfacing. The designated person in-charge stated the slide is no longer used. It was recommended, the center put caution tape or something around the pieces of equipment to ensure it is not used.

Comments:
An unannounced monitoring inspection was conducted on 04/14/2022. At the time of inspection there were 10 children in care with 2 staff present. Children were observed during nap. A sample of 2 staff and 2 children's records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-260-A
Description: Based on a record review the center did not have documentation of an annual fire inspection.
Evidence:
The last documented fire inspection was dated 04/02/2021.

Plan of Correction: The fire department will be called to schedule an inspection.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas/equipment of the center, inside and outside, were maintained in a safe condition.
Evidence:
1. There was peeling paint on the walls in the nap room.
2. The toilet seats in the bathroom located in the nap room had were chipped and the sink bases were also chipped exposing the particle board underneath.
3. One of the toilets in the hallway bathroom was inoperable and the heavy ceramic toilet tank cover was balancing upside down on top of the tank.
4. The kitchen door was rotted at the bottom.
5. The wooden walkway located in the outside play area leading to an exit/entrance had slats that were more than 4inches apart in addition to loose boards.
6. In the outside play area the stairs leading to the back door had 7 risers with no slats between the railings. The railing and door were warped.
7. There was a tile missing in the wall and the foil cover for the AC unit was exposed.

Plan of Correction: Many of these items are on a maintenance list to complete over spring break.
