8VAC20-780 Staffing and Supervision.

An investigation was conducted today regarding a self reported incident involving supervision received on 03/15/2022. Based on staff interviews and video review, the preponderance of evidence supports a violation and the area of non-compliance is identified on the violation notice. Standard #: 8VAC20-780-340-A Description: Based on staff interviews and review of video footage, staff failed to ensure the care, protection, and guidance of one child, when supervising children.
Evidence:
1. On 3/14/2022 a 20 month old child was left unsupervised for approximately 8 minutes in the play area of the outdoor terrace located within the center.
2. When interviewed, Staff #1 and Staff #2 indicated a head count was completed prior to exiting the play area.
3. When interviewed, Staff #1 and Staff #2 stated they became aware Child #1 was not present once they arrived to the classroom and were notified by administration via phone that Child #1 was still in the play area.
3. Video footage of the outdoor terrace play area shows Staff #1, #2, #3 and the remaining children exit the play area, while Child #1 is standing within sight, behind the riding toys along the wall of the building.Plan of Correction: The center has implemented new policies regarding the completion of head counts to include instructing the children to sit together in a designated area before completing the head count. A training was provided to all staff on 4/14/22 to review procedures regarding supervision and head counts.
Standard #: 8VAC20-780-340-F Description: Based on staff interviews and review of video footage, a child under 10 years of age was not always within actual sight and sound supervision of staff.
Evidence:
1. On 03/14/2022, a 20 month old child was left unsupervised for approximately 8 minutes.
2. Staff #1, 2, and 3 left Child #1 alone on the outdoor terrace play area located within the center.
3. Child #1's parent contacted the center after seeing Child #1 on the center's video camera footage, alone on the terrace.
4. Time stamped video footage indicated Child #1 was left alone on the terrace play area while the rest of the staff and children exited the area for 8 minutes. Administrative staff was observed entering the terrace play area to retrieve Child #1.Plan of Correction: The center has implemented new policies regarding the completion of head counts to include instructing the children to sit together in a designated area before completing the head count. A training was provided to all staff on 4/14/22 to review procedures regarding supervision and head counts.
