Comments:
A monitoring inspection was conducted on April 27, 2022. There were 50 children present, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence
The closest in the hallway, which contained hazardous materials-cleaning materials, was not locked making there cleaning materials accessible to children.

Plan of Correction: Closest should have been locked, and was locked during the inspection. We will continually monitor to ensure it is always locked.

Standard #: 8VAC20-780-500-B
Description: Based on observation, staff did not dispose of soiled diapers in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
The diapering surface was not being cleaned with soap and at least room temperature water and sanitized after each use.
Evidence
Staff was observed disposing of a soiled diaper in a leakproof storage system. Staff used their hand to remove the lid to dispose of the diaper.
Staff was observed spraying the diapering surface with a solution of bleach and water after a diaper change. The procedure to following is to spray or dip with a solution of soap and water to clean the surface, pat it dry, spray or dip with a solution of bleach and water and allow to air dry.

Plan of Correction: There is a leakproof storage system that is foot-operated in the bathroom, we will retrain staff to ensure they are using that trash to dispose of the soiled diapers.
We will retrain all staff to ensure they are all following the correct procedure to clean and sanitize the diapering surface.

Standard #: 8VAC20-780-550-P
Description: Based on review of written record of children's serious and minor injuries, the record does not include the name of the parent notified of the occurrence.
Evidence
The current written record of children's serious and minor injuries does not include the name of the parent who was notified of the occurrence.

Plan of Correction: We will redo the forms and include a space to include the name of the parent that was notified.

Standard #: 8VAC20-780-570-E
Description: Based on observation, not all bottles in the refrigerator were not labeled with child's full name and current date.
Evidence
One bottle located in the refrigerator in the infant room, was not labeled with child's full name and current date.

Plan of Correction: Bottle was labeled during inspection. We will ensure all bottles are labled.
