Comments:

An unannounced monitoring inspection was conducted on 4/14/22 from 2:30pm to 4:15pm with the Director. There were 29 children in care, ranging in age from seven-months to five-years-old, supervised by three staff and the Director. The children were observed napping/resting, and waking up and getting ready for snack time. Five child records and two staff records were reviewed. The Center has all staff with current certification in CPR and First Aid, and two staff trained in Daily Health Observation. Three medications and their authorization forms were reviewed and the center has two staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 3/17/22 and the most recent Health Inspection on file was dated 4/1/22. Standard #:	8VAC20-770-60-B
Description:	Based on review of two staff records, the center did not obtain a sworn statement from an employee prior to employment. Evidence: The sworn statement on record for Staff A, with a recorded start date of 3/5/21 and orientation date of 3/4/21, was dated 3/17/21.
Plan of Correction:	Employee accidently used wrong date on Sworn Disclosure form when completing orientation on 3/4/2021. Center Director will confirm that correct dates are written on all orientation documents.
Standard #:	8VAC20-780-80-A
Description:	Based on observation, record review, and interview, the center did not maintain a written record of daily attendance that documented the arrival and departure of each child in care as it occurred. On 4/14/22 at approximately 2:45pm there were 15 children observed in the Two's/Three's classroom. The attendance contained documentation of 16 children present. Interview revealed that one child had left at approximately 12:18pm.
Plan of Correction:	Walnut Grove Child Care Center has begun implementing a new written attendance tracking policy. Upon arrival to the center site director and classroom teacher will initial off on attendance clip board with the time child arrived. Upon picking up center site director and classroom teacher will initial off on attendance clip board with the time child is leaving the facility.

Description:	Based on observation and interview, the center did not ensure that the ratio of 1:4 (staff:children) for children birth up to 16 months was maintained at all times. Evidence: At approximately 2:35pm on 4/14/22 five children were observed with one staff in the Infant room. The youngest child in attendance was seven-months-old.
Plan of Correction:	Due to the critical nation-wide labor shortage Walnut Grove Child Care Center is working diligently to hire new qualified staff while also complying with all COVID-19 protocol and standards currently in place by the VDH and the VDOE.
Standard #:	8VAC20-780-350-B-2
Description:	Based on observation and interview, the center did not maintain the required ratio for children 16-months to 24-months-old. Evidence: 1. On 4/14/22 at approximately 2:40pm, one staff was observed in the "Two's/Three's" classroom supervising 15 napping/resting children. The youngest child in attendance was 21-months-old. During the designated rest period with a 21-month-old present, the required staff to child ratio was 1:10. 2. On 4/14/22 at approximately 3:00pm, 2 staff were observed supervising 15 children in the "Two's/Three's" classroom. The youngest child in attendance was 21-months-old and required a ratio of 1:5.
Plan of Correction:	Due to the critical nation-wide labor shortage Walnut Grove Child Care Center is working diligently to hire new qualified staff while also complying with all COVID-19 protocol and standards currently in place by the VDH and VDOE.
