Technical Assistance:
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

Comments:
A monitoring inspection was initiated on 5/5/22 and concluded on5/5/22. There were 34 children present, ranging in ages from infant to five, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to obtain all the required information for one out of three children's records.

Evidence:

1. The record for child 3 did not contain the home and work phone numbers for the father of the child.
2. The director stated they did not have the phone numbers for the child.

Plan of Correction: The missing information for child 3 will be obtained and added to the child's record. In the future children's records will be reviewed to ensure all required information has been provided prior to the first date of attendance.

Standard #: 8VAC20-780-550-P
Description: Based on record review, the center failed to maintain a written record of children's injuries that include the date the parent was notified.

Evidence:

Three injury reports were reviewed. One report did not contain the date the parents were notified of the injury.

Plan of Correction: Staff will be retrained to ensure the entire injury report document is completed.
