Technical Assistance:
Reviewed documentation of annual training and program leader qualifications for staff.

Comments:
An unannounced monitoring inspection was conducted on 05/03/2022 from 4:10 pm - 7:00pm. At the time of entrance there were 24 school age children in care with five staff supervising. Children were observed during outdoor play and working on homework and projects in the science classroom and the multipurpose classroom. Records were reviewed for three children and seven staff.
Violations were found in the areas of physical plant and supervision of children. These violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas of the center shall be maintained in a safe condition.

Evidence:
Areas of peeling paint observed on the lower wall beside the toilet of the basement bathroom.

Plan of Correction: Maintenance will be contacted immediately to address this area and make repair.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Unlocked chemicals and cleaning agents were observed in the following areas:
a. A bottle of peroxide disinfectant and Windex was observed on the book case in the multipurpose classroom;
b. Three cans of spray paint/primer and one can of rust oleum were stored on the book case in the science classroom;
c. A can of Lysol spray was observed on top of the paper towel dispenser in the basement bathroom.

Plan of Correction: All chemicals were locked up during the inspection. Staff will be reminded to lock up chemicals when children in the extended care program are in care.

Standard #: 8VAC20-780-340-F
Description: Based on observation, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence:
1. Child 1, age 6 years, was observed in the multipurpose classroom without any adult supervision for approximately 2 minutes at 4:20 pm.
a. Child 1 was observed walking into the classroom from the playground while staff remained on the playground.
b. The location of this classroom does not allow for sight or sound supervision of children by staff on the playground.
2. Child 2, age 7, was observed in the second floor hallway without any adult supervision for approximately 30 seconds at 4:35 pm. No adult supervision was present while this child was walking through the hallway.

Plan of Correction: All staff in the extended care program will be reminded that children can not enter and exit classrooms and outdoor play areas without adult supervision.
