Technical Assistance:
Discussed requirements for expired medication.

Discussed and emailed provider information about changes in regulations which occurred on 10/13/2021.

Comments:
An unannounced monitoring inspection was conducted on May, 5, 2022. The physical plant, 4 staff records, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside, and eating lunch from home and a school supplied snack celebrating the Cinco de Mayo holiday. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, 1 of 3 children's records reviewed did not contain all required information.
Evidence:
Child 1's record did not contain the addresses for two persons to contact in the event a parent cannot be reached.

Description: Based on observation, not all outlets had protective covers installed.
Evidence:
Outlets without protective covers were observed in the hallway areas of the area of the recreation center that is used for licensed child day programs.

Description: Based on staff statement, the center did not keep a record of all medications given to a child to include the child's name, the amount and type of medication, the date and time of administration, the name of administering staff member, or any adverse reactions to the medication.
Evidence:
A staff member stated that she recently administered authorized medication to Child 4, but did not document the occurrence.
