An unannounced complaint inspection was conducted on 3/03/2022 and concluded on 5/10/2022 regarding care of children. Based on interviews conducted and documentation reviewed, the incident is deemed valid. Violations cited for care of children and are identified in the Violation Notice.

Standard #: 8VAC20-780-270-A Complaint related: No Description: Evidence: Observation of the classroom of three year old children located on the lower level of the center had a water leak which had odor that could be smelled as you enter the center. The carpet in the classroom was wet due to the water leak. Upon arrival in the classroom, the windows were closed upon entry in the classroom and there was no equipment use to help with the odor in the classroom. Plan of Correction: Repaired leak, used air purifier, cleaned floor, walls and replaced carpet
