Comments:
A monitoring inspection was initiated on 5/6/2022 and concluded on 5/10/2022. There were 35 children present, ranging in ages from 18 months to 12 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 0 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on review of five children's records, the facility failed to have documentation of a physical within 30 days of enrollment. Evidence: Child 3 (date of enrollment 2/7/2022) did not have documentation of a physical on file.

Plan of Correction: Center staff will get the information from parents.

Standard #: 8VAC20-780-60-A
Description: Based on review of five children's files, the facility failed to have all required information in each child's file. Evidence: Child 2 did not have documentation of the name and phone number of the child's physician, the name, address, and phone number of a second emergency contact, or any reported allergies. Child 4 did not have the name, address, and phone number of a second emergency contact.

Plan of Correction: Center staff will obtain the missing information from parents.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the facility failed to maintain all areas in a clean, safe, and operable condition. Evidence: there were ants in a pack of donuts that were served to one child. The lights in the hallway to the bathrooms were dim, making it hard to see. The outlet cover plate in one bathroom was missing. There was a strong chemical/cleaner odor in the center; center staff stated it was from having the floors waxed.

Plan of Correction: The center will be treated for ants. The outlet plate will be replaced. The lights will be adjusted. The center will be aired out.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the facility failed to have all hazardous substances, such as cleaning materials, in a locked place. Evidence: there was Clorox spray cleaner and Great Value all purpose cleaner in an unlocked cabinet in the diaper changing area. There was a gallon of windshield wiper fluid on the open lower shelf of the diaper changing table.

Plan of Correction: Hazardous substances will be locked.

Standard #: 8VAC20-780-350-B-2
Description: Based on observation, the facility failed to maintain the required ratio of one staff for every 5 children ages 16 months to two years. Evidence: on 5/6/2022, there were 6 children ages 18 months to 2 years old with one staff.

Plan of Correction: A new staff person is in the process of being hired.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation, the facility failed to maintain the required ratio of one staff for every 10 children ages 3 years old to school age. Evidence: on 5/6/2022, there were 24 children, ages 3 to 12 years, present with only two staff.

Plan of Correction: A new person is in the process of being hired.
