Comments:
A renewal inspection was conducted on 05/05/2022, from 11am to 1:18pm. There were 39 children present and 11 staff. The Inspectors reviewed five children?s records and three staff records. The Inspectors observed the interaction of staff and children throughout the inspection. Children were observed during free play, circle time, toileting, handwashing, lunch and feeding, and naptime. First aid kits, and emergency preparedness documents were observed. Transportation vans were viewed for compliance. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the program director and assistant director at the exit interview.

Violations:
Standard #: 8VAC20-780-160-A-2
Description: Based on record review and interview, it was determined that the tuberculosis screening documentation was not documented within the last 30 calendar days of the date of employment.

Evidence:

1) Staff #2 (date of hire 03/28/2022) has a tuberculosis screening with the date of 09/06/2021 which is not within 30 days prior to employment.
2) The center director, Staff #1, confirmed that a tuberculosis screening was not completed by the required timeframe.

Plan of Correction: The center responded with the following:
The center will ensure that all newly hired staff will complete a tuberculosis screening within 30 days prior to their date of hire.

Standard #: 8VAC20-780-160-C
Description: Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:

1) Staff #3 (date of hire 07/29/2013) has a tuberculosis screening with the date of 12/21/2019 which is not within the last two years.
2) Administration confirmed that the TB record for Staff #3 was not within the past two years and acknowledged that a new TB screening was required.

Plan of Correction: The center responded with the following: Administration will inform Staff #3 of the required tuberculosis screening and to submit documentation to the center.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, it was determined that the center did not ensure that hazardous substances such as cleaning materials were locked in a place using a safe locking method that prevents access by children.

Evidence:

1) The Pre K classroom has an aerosol disinfectant spray can that on the label had a precautionary statement, ?Hazards to Humans,? that is unlocked in a drawer accessible to children.
2) The girl?s bathroom has a tube of diaper cream in reach of children.
3) The center director, Staff #1, confirmed these hazardous substance were accessible children.

Plan of Correction: The center responded with the following: Staff #1, Director, placed the spray can in a locked cabinet during the time of the inspection. The tube of diaper cream was placed on a high shelf out of reach from children and will be kept in a box with a safe locking method.

Standard #: 8VAC20-780-440-L
Description: Based on observation and interview, it was determined that the center did not ensure that pillows and filled comforters shall not be used by children under two years of age.

Evidence:

1) While children were sleeping in their cribs in the infant room, four of the eight children under the age of 12 months, were observed with blankets and/or filled plush toys in their cribs.
2) The center director, Staff #1, confirmed the blankets are routinely used inside the crib.

Plan of Correction: The center responded with the following: Staff will remove all blankets out of the crib. The center will inform parents that an alternate method such as a sleep sack would be more appropriate. The center will inform parents that if a blanket is desired, only a birthing or other loosely woven blanket through which an infant could easily breather would be allowed.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview, it was determined that the center did not ensure that a nonabsorbent surface for diapering or changing be used.

Evidence:

1) The diaper changing pad on the changing table in the infant room has a tear making it an absorbent surface. Infant room staff were observed changing diapers on this pad.
2) The center director, Staff #1, observed and confirmed the tear.

Plan of Correction: The center responded with the following: The Director will purchase a new diaper changing pad and dispose of the old changing pad.

Standard #: 8VAC20-780-570-A
Description: Based on observation and interview, it was determined that the center did not ensure that when a child is placed in an infant seat, high chair, or feeding table that the protective belt is fastened securely.

Evidence:

1) Two children in the toddler room that were sitting in their high chair were observed to have no protective belts fastened.
2) A child that finished eating lunch at a feeding table in the toddler room was observed to have no protective belt fastened.
3) Staff #1 and the classroom teachers confirmed that the children did not have their protective belts fastened while sitting in their seats to eat their lunch.

Plan of Correction: The center responded with the following:
All staff will be reminded that protective belts must be fastened at all times when a child is in an infant seat, high chair, or feeding table. Missing belts were replaced during the inspection.

Standard #: 8VAC20-780-570-B
Description: Based on observation and interview, it was determined that the center did not ensure that bottles are not be used while the child is in his designated sleeping location.

Evidence:

1) The infant room had three of the eight children with feeding bottles in their cribs.
2) Staff #1 and the infant teachers in the classroom confirmed that bottles were being used by the infants while they are in their designated crib.

Plan of Correction: The center responded with the following:
Infant teachers immediately removed the bottles from the cribs during the inspection. Infant room staff will ensure proper feeding techniques for the children as required by licensing standards.
