Comments:
An unannounced renewal inspection was conducted on-site on April 19, 2022 and concluded remotely on April 21, 2022. The director was available during the inspection. There were 24 children present, ranging in ages from 2 months to 5 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance
from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-180-A
Description: Based on observation and interview on April 19, 2022, a staff member was not capable of carrying out assigned responsibilities.
Evidence: 1. A staff member was observed working alone in a group of 5 infants. The staff
member was observed having difficulty walking and rollator walker belonging to that staff was
observed just outside the gate to the infant room. The staff appeared to be unable to walk
steadily in the infant area and had to hold on to stationary furniture for support. The staff was
observed to be unable to pick up an infant that was laying on his back under a table. The staff
was able to move the child out from under the table using her hand and then turned the infant
over unto his stomach. 2. When asked if the staff was able to pick up the infants the staff stated "I would never pick up a baby knowing I can't." The staff member did state she was able to move the infants from the feeding table to the floor.

Plan of Correction: 1. We will replace said staff member with a new hire and assistant. 2. This will be monitored by the owner/director and the new hire will begin work as of June 5, 2022. 3. The details of this IPOC will be monitored under direct supervision of the owner/director.

Standard #: 8VAC20-780-280-B
Description: Based on observation on April 19, 2022, the center did not ensure that hazardous substances
such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence: In the infant room a container of Clorox wipes were kept on top of the diaper changing table. The label on the container of wipes states "keep out of reach of children" and
"caution."

Standard #: 8VAC20-780-350-B-1
Description: Based on observation and interview on April 19, 2022, the center did not ensure to maintain the required staff-to-children ratio of 1 staff to 4 children in the infant room (ages birth to 16
months).
Evidence: The infant room had 5 infants present with 1 staff supervising. The youngest infant in the room was child #4 (age 2 months old). Administration acknowledged they were out of
ratio.

Plan of Correction: 1. We will no longer offer Pre-K program, as we have applied for the ratio increase and failed to receive a response and hiring staff to accommodate the preschool classroom has not seemed to be a realistic goal within the current times of finding employees willing to work and lack of interested candidates has made it impossible to keep operating all classrooms. We will close the preschool program and move staff to accommodate our infants-three year old classroom moving forward. The Pre-K parents will be notified as of June 1, 2022 and the classroom will be closed as of July 15, 2022. We will continue to work on the hiring process and to find suitable candidates to work in the center alongside our current staff. 2. The hiring and release of the 4-year old program will be overseen by the owner/director. The details of this IPOC will be monitored under the direct supervision of the owner/director.

Description: Based on observation, children's record review and interview on April 19, 2022, the center did
not ensure to maintain the required staff-to-children ratio of 1 staff to 5 children in the toddler
room (ages 16 months to 24 months).
Evidence: There were 6 children present in the toddler room with 1 staff supervising. The
youngest child in the toddler room was identified by staff #2 as 16 months old (child #1). After
a review of the child's #1's record it was determined that child #1 was 14 months old. Administration acknowledged the classroom was out of ratio.

Plan of Correction: 1. We will no longer offer Pre-K program, as we have applied for the ratio increase and failed to receive a response and hiring staff to accommodate the preschool classroom has not seemed to be a realistic goal within the current times of finding employees willing to work and lack of interested candidates has made it impossible to keep operating all classrooms. We will close the preschool program and move staff to accommodate our infants-three year old classroom moving forward. Description: Based on observation and interview on April 19, 2022, the center did not ensure to maintain the required staff-to-children ratio of 1 staff to 10 children in the preschool room.
Evidence: There were 13 children present with 1 staff supervising in the preschool room.
Administration acknowledged the classroom

Plan of Correction: 1. Description: Based on observation and interview on April 19, 2022, the center did not ensure that staff wash their hands with soap and running water before serving food and before a diaper change, and they did not ensure that children wash their hands with soap and running water after a diaper change and after eating meals or snacks.
Evidence: 1. Staff #1 put on gloves but did not wash her hands before serving breakfast. 2.
Children in the toddler and preschool classrooms did not wash their hands after eating
breakfast and immediately started playing with toys. 3. Staff #2 put on gloves prior to a diaper
change but did not wash her hands. Staff #2 did not wash her hands after diapering the child.
Staff #2 did not wash the child's hands after diapering and stated she only does if the child
touches themselves during the change. 4. Administration acknowledged handwashing is
not being done properly.

Description: Based on observation and interview on April 19, 2022, the center did not ensure to follow the
most recent, age-appropriate nutritional requirements of a recognized authority such as
the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence: Children were not served fluid milk at breakfast. Administration stated they were short staffed and no one could go get it and acknowledged the children didn't receive it.

Standard #: 8VAC20-780-560-J
Description: Based on observation and interview on April 19, 2022, the center did not ensure tables and high chairs are sanitized before and after each use for feeding.
Evidence: 1. Staff in the 3 year old class was observed to spray sanitizing solution on the
table after breakfast and immediately wipe the solution off. 2. Staff in the toddler room was
observed to spray high chairs and the table with sanitizing solution and immediately wipe it
away. 3. Administration acknowledged the staff are not waiting 2 minutes for the solution to air
dry.

Description: Based on observation and interview on April 19, 2022, the center did not ensure when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.
Evidence: 1. Child #1 (age 14 months) and child #2 (age 16 months) were observed not secured in their high chairs during breakfast. 2. Staff #2 acknowledged they were not strapped in. 3. Two infants were observed not secured in the feeding table in the infant room. 4. Staff #4 confirmed the infants were not secured in the feeding table.
