Comments:
A renewal inspection was conducted on 5/16/2022 9:35 a.m. ? 4:46 p.m. There were 149 children present ranging in ages from 5 months to preschool age and 24 staff supervising. The inspectors reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition, and transportation. A total of 10 child records, 3 medication records, and 15 staff records were reviewed. The information gathered during the inspection determined violations with applicable standards. A violation notice was issued.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure that all areas and equipment of the center, inside were maintained in a clean, safe, and operable condition.

Evidence: The toilet seat and seat cover in the Explorers Room bathroom used by children showed cracks and flaking of the water resistant covering allowing liquids to soak into the underlying porous materials.

Plan of Correction: Explorers toilet seat was replaced and pictures sent to inspector on the same business day as inspection.

Standard #: 8VAC20-780-330-B
Description: Based on observation and interview, the facility did not ensure that all playground equipment in the toddler playground area was provided the minimum amount of resilient surfacing to comply with the minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials.

Evidence: Measurements taken around the swing and climbers showed between 4.5 and 5 inches of resilient surfacing when at least 6 inches of surfacing was required based on the height of the equipment. The program director acknowledged the need for additional resilient surfacing.

Plan of Correction: The toddler playground surfacing was already scheduled to undergo renovation from playground mulch to a turf surfacing. This will include excavation and removal of all current playground mulch and replacement with appropriate padding/resilient surfacing and turf covering. Playground is closed until renovations completed.
