22.1 BACKGROUND CHECKS, CODE, CARBON MONOXIDE

Comments:
An unannounced monitoring inspection was conducted on 05/18/2022. There were 25 children, 12 months-5 years, and 9 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in free choice activities and during story time. A total of 5 children?s records and 5 staff records were reviewed. The inspector arrived for the inspection at 9:15 am and departed at 1:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and discussion with staff, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. The trees behind the fence line along the play area were overgrown and hanging over various pieces of play equipment.
2. There was a wasp?s nest in the Little Tykes plastic play house on the playground.
3. The wooden boarder around the small sandbox on the lower end of the play area was deteriorated and had a large hole in it.
4. The mesh lining inside of the small sandbox on the lower level was visible and was a tripping hazard.
5. There is a dead tree trunk inside the play area that has a sharp broken branch that is 3 foot 5 inch from the ground; the sharp branch could snag or impale skin.
6. The fence at the lower level of the play area is broken and rusted.

Plan of Correction: Will get someone to cut tree removed. Will tape off the other side of the playground. Will get the wasp out and cut the bushes.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation and discussion, the center failed to ensure that in areas of centers serving preschool age or younger, electrical outlets shall have protective covers.

Evidence:
1. There was an uncovered electrical outlet in the area of the large room used by the Pre-K group.
2. There were 2 uncovered electrical outlets in the play room used by the children.
3. The children in care were preschool age and younger.

Plan of Correction: Were covered during the inspection.
