Comments:
An unannounced monitoring inspection was conducted on site on 5/3/2022. The inspector arrived at 9:00 AM and departed at 10:30 A.M. There were 12 children present with 3 staff. The inspector reviewed five children?s records and 2 staff. The inspector observed the children outside, circle time outside and handwashing . Discussion with director about out of state background checks.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of five children?s records, the center failed to ensure that all records were complete per the standard.

Evidence:
1. Child # 1 was missing allergies and actions to be taken in case of the allergy.
2. 2. Child # 5 was missing the Physician?s Name and phone number.

Plan of Correction: The administration will have the parent to complete the missing information ASAP.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the center failed to ensure that all playground equipment had the required amount of resilient surfacing.

Evidence:
1. Directly under the swings and climbing equipment there was not enough resilient surfacing to measure. The required amount would be 6 inches.

Plan of Correction: The school has ordered additional resilient surfacing and should be delivered soon. Staff will monitored all areas as stated in the playground safety plan.
