Comments:
A monitoring inspection was conducted on 05/04/2022. There were 108 children present, ranging in ages from 2 to 5 years. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based upon review of 5 staff records and staff interview, the facility has not ensured that documentation of a negative tuberculosis screening is obtained for all staff.
Evidence:
1. The record provided for staff 3, employed on 3/30/2021, did not include documentation of a negative tuberculosis screening.
2. Staff 1 verified that the results of a negative tuberculosis screening were not in the record for staff 3.

Plan of Correction: The facility responded with the following:
The employee was contacted during the inspection to provide the required tuberculosis screening documentation. All staff records will be reviewed to ensure that all requied documentation is included in each record.

Standard #: 8VAC20-780-40-M
Description: Based upon observation and staff interview, the facility has not ensured that a current written list of all children's allergies, sensitivities and dietary restrictions is dated and kept confidential in each room or area where children are present.
Evidence:
1. There was a listing of children's names and their allergies posted in the hallway outside of the two year old classroom. The list was not dated and kept in a way to be confidential.
2. There was no listing of children's allergies available to staff in room 15, a four year old classroom.
3. Staff 1 acknowledged that the listing in the hallway was not dated and kept in a confidential fashion.

Plan of Correction: The facility responded with the following:
The children's allergy chart was dated, printed and distributed to all classrooms during the inspection. Policies/procedures will be developed to ensure that allergy charts are kept confidential but available in all areas where children are in care. All staff will have access to the allergy charts at all times.

Standard #: 8VAC20-780-510-I
Description: Based upon review of medication documentation, the facility has not ensured that medication stored for emergency administration to children is accompanied by written parent authorization for the medication to be administered.
Evidence:
There was an albuterol inhaler, benadryl and an epipen stored for emergency administration to child 6. The parent authorization forms for these medications to be administered were not signed by a parent.

Plan of Correction: The facility responded with the following:
The parent of child 6 was contacted during the inspection when it was discovered that the parent had not signed the medication authorization forms. The parent arrived and signed the forms during the inspection. All medication documentation has been reviewed to ensure that it is complete. In the future, care will be taken to ensure that all required information/authorizations are obtained prior to accepting any medication at the facility.
