Comments:

A monitoring inspection was conducted on 5/23/2022 with center director. There were 16 children present, ranging in ages from 3 years to 5 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed. The children were dancing to music and practicing for graduation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22.1-289.035-B-2
Description:	Based on review of four staff files and staff interview, the center failed to ensure that four staff obtained fingerprint background check results prior to employment. Staff 1's date of hire was documented as 8/2019; the fingerprint results were dated 10/9/2020. Staff 2's date of hire was documented as 10/28/2021; the fingerprint results are dated 3/3/2022. Staff 3's date of hire was documented as 10/28/2021; the fingerprint results are dated 5/2/2022. Staff 4's date of hire was documented as 8/29/2019; the fingerprint results are dated 10/07/2020. The center director stated the fingerprint results were not obtained prior to employment
Plan of Correction:	Staff 1 and Staff 4 did not have fingerprint checks on file when the director took over the files. The staff were sent as soon as possible. Staff 2 and Staff 3 submitted to fingerprints, but the results were never processed. They went back a few months later. All staff will obtain satisfactory fingerprint checks prior to hire.

Description:	Based on review of four staff files, the facility failed to ensure that each staff completed a sworn disclosure statement prior to employment as required. Evidence: Staff 1 (date of hire 8/2019) did not have a completed sworn disclosure statement until 9/21/2020.
Plan of Correction:	All staff will complete a sworn disclosure statement prior to employment.

Description:	Based on review of four staff files, the facility failed to ensure that each staff had a search of the central registry background check (CPS) within 30 days of employment as required. Evidence: Staff 1 (date of hire 8/2019) did not have a completed search of the central registry until 10/1/2020.
Plan of Correction:	All staff will send a search of the central registry within 7 days of employment.

Description:	Based on review of four staff files, the center failed to obtain documentation of a tuberculosis (TB) test or screening at the time of employment and prior to coming into contact with children. Evidence: Staff 2 (date of hire 10/28/2021) did not obtain a TB test or screening until 3/2/2022.
Plan of Correction:	All staff will obtain a TB test or screening before coming into direct contact with children.
