Comments:
An unannounced mandated inspection was completed on 05/19/2022 from 9:30 a.m.-1;35 p.m. At the time of the inspection eighty-eight children were in care with twenty staff present. Nine children records, nine staff records, five medication authorizations, required documentation, health inspection, fire inspection, and posted required information were reviewed and updated. Children and staff were observed during free indoor play, diapering, art activities, clean-up, hand washing, lunch preparations, lunch, lunch clean-up, nap, transitions and behavioral guidance. The violations from the previous inspection were reviewed for current compliance. The exceptions to compliance are noted on the violation notice.

Description: Based on medication review, the center did not ensure five children's records contained documentation of a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
1. The records of Child #1, Child #2, Child #3, Child #4, and Child #5 did not have documentation of a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
2. Administration confirmed there was not a written care plan for each child with a diagnosed food allergy.

Plan of Correction: I am going to request the parents get the correct allergy action plans to have on file.

Standard #: 8VAC20-780-510-I
Description: Based on medication review, the center failed to ensure that prescription medication was maintained in the original labeled container.

Evidence:
1. One prescription medication, for Child #1, was not stored in current container. Box expiration date: 12/2021
2. Medication expiration date: 06/2022
3. The staff confirmed the container date of 12/21 and the medication date 06/22.

Plan of Correction: The teacher will tell the parent to bring in the original box for the medication.
