An unannounced monitoring inspection was conducted on 6/2/22 from 12:35pm to 2:15pm with the Director and Assistant Director. There were 45 children in care, ranging in age from three-months to 10-years-old, supervised by nine staff. The children were observed having free play, finishing lunch, getting ready for nap while reading books on cots, and using the restroom prior to nap. Lunch served was pasta salad with ham and spinach, applesauce, and bread. Six child records and three staff records were reviewed. All center staff have current certification in CPR and First Aid as well as Daily Health Observation Training. One medication and authorization form was reviewed and the program has seven staff with current certification in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent fire inspection on file was dated 11/18/21 and the most recent health inspection on file was dated 3/10/22. Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on 6/2/22 the Licensing Inspector observed two disinfectant spray bottles on a high shelf over the sink in the four-year-old classroom.

Plan of Correction: The spray bottles were moved to a locking cabinet during the inspection.

Standard #: 8VAC20-780-430-H
Description: Based on observation, the center did not ensure that trampolines were not used. Evidence: An indoor trampoline was observed in the Schoolage classroom on 6/2/22.

Plan of Correction: We will remove this today.

Standard #: 8VAC20-780-440-B
Description: Based on observation and interview, the center did not ensure that cribs were identified for use by a specific child. Evidence: During the inspection on 6/2/22, there were five infants in care in the Infant Class. There were only four cribs in the Infant Class. Interview revealed that the infants share cribs with the sheets changed in between.

Plan of Correction: We have more cribs in storage. We will have another put together and added to the classroom by Monday, 6/6/22.
