Comments:
An unannounced monitoring inspection was conducted on June 6, 2022. The inspectors arrived at 10:00 AM and departed at approximately 12PM. There were 18 children present and seven staff. The Inspectors reviewed five children?s records and five staff records. The Inspectors observed staff and children?s interactions throughout the inspection. Children were observed during outdoor play, free play with music, bottle feeding and snack. Diapering and handwashing were observed. First aid kits, emergency preparedness documents, and the transportation vehicle were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the center?s director at the exit interview.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on record review and interview, it was determined that center did not ensure that at least every two years from the date of the initial screening or testing that a staff member obtain and submit the results of a follow-up tuberculosis screening.
Evidence:

1. The record for Staff #2 contain a tuberculosis screening that is past due for update.
a. The most recent tuberculosis screening for Staff #2 was dated March 8, 2020.
2. The center?s director acknowledged that a new TB screening was required two years from the date of the last screening and documents were not available in the record of Staff #2 to indicate that it had been completed.

Plan of Correction: The center responded with the following: Staff #2 will have the TB screening completed by the end of the week and have documentation placed in the staff?s record. An email to the licensing inspector will be sent on the day of completion.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that the areas and equipment of the center be maintained in a safe and operable condition.
Evidence:
1. In the playground, there are three small plastic toddler bikes with cracks on the handlebars and seats posing as a pinching hazard to the children. Children were actively playing in the playground and on the bikes.
2. The plastic table on the playground has a 3 inch crack on the edge of the table. There were two toddlers sitting at the table during the observation.
3. In the playground, there are four large metal screws attached to a mulch barrier approximately one inch in diameter and 4-6 inches long protruding from the ground. This poses a tripping and injury hazard for the children.
4. The protective lattice gate around the air conditioning unit was torn down and on the ground. This poses a tripping hazard and allows access to a running AC unit.
5. The playground deck?s wooden stairs has an approximately 6-8 inch missing piece of wood on one of the steps.
6. The toddler room had three small children?s couches that has torn material throughout the seat area that compromises the couch?s function as a chair. The rip in the material exposes the wood framing of the couch in which children can hurt themselves when sitting on the couch.
7. Staff # 1 confirmed the cracks on the bikes and tables in the playground as well as the torn couches in the toddler room. Staff #1 acknowledged the protruding screws and broken lattice gate.

Plan of Correction: The center responded with the following: Staff will dispose of the cracked bike and table by June 7, 2022, and speak to the owners about replacements. Staff #1 will speak to the owners of the center about putting in work orders for the playground to be fixed and per licensing standards as soon as possible.

Standard #: 8VAC20-780-280-B
Description: During observation and interview, it was determined that the center did not ensure that hazardous substances such as cleaning materials were locked in a place using a safe locking method that prevents access by children.
Evidence:
1. In the infant room, with six children present, there was one bottle of bleach and water and one bottle of disinfectant and water under the sink unlocked.
2. The changing table cabinet in the toddler room had an air freshener aerosol spray can that indicated to keep out of reach of children.
3. Staff #3 confirmed that the bottles in the infant room were not securely locked and are accessible to children.
4. Staff #1 confirmed the aerosol spray can was unlocked and accessible to children.

Plan of Correction: The center responded with the following: Infant staff will ensure that all bottles kept under the sink are securely fastened with the cabinet locked at all times. The aerosol can was immediately removed and locked at the time of the inspection.

Standard #: 8VAC20-780-290-A-2
Description: Based on observation, it was determined that the center did not ensure that fans be kept out of reach of children.
Evidence:
1. In the infant room there is a large 20 inch fan placed on a low shelf that is routinely used and is within reach of children when crawling and walking.
2. Staff #3 and Staff #5 confirmed that the fan is in use while infants are in care and that it is in reach of children.

Plan of Correction: The center responded with the following: The fan was removed from the infant room during the time of the inspection. Infant room staff will not use the fan until administration has the fan secured in the room and high enough that it is out of reach of children.

Standard #: 8VAC20-780-440-J
Description: Based on observation and interview, it was determined that the center did not ensure that there is thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib.
Evidence:
1. In the infant room, there is a crib assigned to an infant in care that is placed between the sink and rocking chair in which there is no space between the service sides.
2. Staff #1 confirmed that the crib did not have the required thirty inches of space between the service sides of the crib.

Plan of Correction: The center responded with the following:
Staff will rearrange the cribs to ensure appropriate spacing between cribs and furniture.

Standard #: 8VAC20-780-510-L
Description: Based on observation and interview, it was determined that the center did not ensure that medications are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the infant room the medication for Child #1 was unlocked in the bathroom used to change children.
2. Staff #3 confirmed that the medication was not locked.

Plan of Correction: The center responded with the following: Staff #3 and all staff members in the facility will keep all medications locked using a safe locking method.
