Technical Assistance:
Technical assistance was offered related to: infant feeding schedules, children's allergies and staff records.

Comments:
An unannounced renewal inspection was conducted today from 10:25am to 12:45 pm. There were 43 children in direct care and supervision of 12 staff members. The children were observed engaged in activities including but not limited to playing with floor toys, going for an outdoor walk, singing songs and eating lunch. The lunch menu included rice with meatballs, steamed carrots and milk. A discussion was held with the staff in charge that staff for whom background checks are being obtained must ensure that all information is accurate on the forms.
3 children's records, 4 staff records, attendance records, emergency drill records, and children's medications were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-B-4
Description: Based on staff interview and records review, the center did not obtain for an employee that lived outside of Virginia within the last 5 years, results of an out of sex offender search and out of state criminal record check (CRC) prior to their first day of employment; and the results of an out of state central registry search within 45 days of employment.
Evidence:
1. Staff # 1's first day of employment was 2/9/22. Staff # 1's record had written information that indicated that she lived in Texas within the last 5 years.
2. This information was not documented on her Sworn Disclosure Statement (SDS).
3. Upon interviewing, Staff # 1 stated that she moved to VA in December 2021 from Texas. When asked about the SDS, Staff # 1 stated that she did not complete the SDS, and someone from the center did and that they may have gotten confused.
4. The results of the CRC and sex offender search for Texas were not obtained prior to Staff # 1's first day of employment.
5. The central registry search for Texas was not submitted and the results were not obtained within 45 days of employment.

Plan of Correction: We have completed and submitted the required background checks on staff # 1 to the pertinent authorities.

Standard #: 8VAC20-780-340-D
Description: Based on records review and staff interviews, the center did not have in each grouping of children at least one staff member who meets the qualifications of a program leader or program director regularly present.
Evidence:
1. There were 5 staff present in the Infant classroom. Staff # 2 stated that currently there was not a program lead qualified staff in this classroom.
2. There were 2 staff present in the Crawlers (9 months to 16 months of age) classroom. Staff # 2 stated that neither staff was program lead qualified.
3. There were 4 staff present in the 2's classroom. Staff # 2 stated that there was not a program lead qualified staff in this classroom.

Plan of Correction: As we all know the workforce is minimal. We have been in the process for the last two years and continue to be in the interviewing process to find qualified candidates that meet the state requirements.

Standard #: 8VAC20-780-570-I
Description: Based on staff interview and observation, the center did not maintain one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care at the center.
Evidence:
1. In the infant classroom, it was observed that the infants had their daily supply of milk bottles which included breast milk for some and formula for others.
2. One day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care shall be maintained at the center.
3. Staff # 2 stated that sometimes parent s bring an extra bottle or so, but there were not enough bottles to meet one day's supply and some infants only had breast milk at the center.

Plan of Correction: We have notified families about bringing in extra milk/formula to be kept at school for emergency situations.
