Comments:
An investigation was initiated on 6/13/2022 regarding a self reported incident involving behavioral guidance and forbidden actions. Interviews were conducted and documentation was reviewed. The investigation regarding behavioral guidance and forbidden actions is determined to be valid.

Violations:
Standard #: 8VAC20-780-400-B
Description: Based on interviews and documentation review it was determined that behavioral guidance was not always appropriate and constructive in nature. Evidence: It was reported that on 4/26/2022 Staff A placed child A (Age 4) in a bathroom with the lights off because they did not finish their lunch. It was reported that the child was screaming and crying while in the bathroom and proceeded to urinate on themselves.

Plan of Correction: Upon internal notification of the incident by a staff member, The Goddard School immediately notified VDOE and other agencies, as well as the parents of Child A. The Goddard School assisted with the VDOE investigation. Upon conclusion of the School?s internal investigation, the School terminated Staff A. At all relevant times, all other children within the School?s care were safe and secure, and no children are or were at risk for any inappropriate handling.

The Goddard School does not and will not tolerate the behavior described in the violation notice. Proper professional conduct is outlined in the Goddard School Employee Handbook. On May 18, 2022, and June15, 2022, all staff attended mandatory training sessions on the practices for the appropriate handling of
children. The trainings included role-playing to ensure that appropriate care techniques and proper handling behaviors are understood and practiced by all staff. The School strives to ensure that professional, quality education and childcare in a safe, loving nurturing environment. The School will continue to work with its staff to implement the corrective actions outlined in our Plan of Correction to reinforce appropriate, nurturing childcare.

Description: Based on interviews and documentation it was determined that staff roughly handled a child which is a forbidden action. Evidence: On 5/11/2022 it was reported that staff A yanked the arm of child B while washing their hands in the restroom. It was reported that child B (Age 3) began to cry as a result of the incident.

Plan of Correction: Upon internal notification of the incident on May 11, 2022 by a staff member, the Goddard School immediately notified VDOE, and cooperated with the VDOE investigation. Upon conclusion of the School?s internal investigation on said day, the School terminated Staff A. This incident was a single incident between Staff A and Child B.
