Discussed the requiremtns for resilent surfacing under the climbing wall and balance beam, ideas to have area for children's belongings and looked at the DHO training that was used for there site.
- Comments:

An unannounced monitoring inspection was conducted on June 16, 2022. At the time of entrance there were 115 children and 13 staff members present with children. The sample size consisted of 5 children's records and 5 staff records. Children were observed outside playing games, inside eating snack and preparing for ttansition to the next activity. The inspection began at approximately 11:15am and ended at approximately 2:02pm. Areas of non-compliance are identified on the violation notice.

Standard #: 8VAC20-780-160-A-2 Description: Based on recorrd review, the licensee did not ensure the documentation have been completed with the last 30 days of employment .
Evidence : Staff #3 ( hire date 05/25/2022) and Staff #5 (hire date 06 /13/2022 ) TB test were completed more than 30 days before employed. The TB teste were completed in March 2022.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-80-A Description: Based on record review and staff interview, the licensee did not ensure for each group of children , the center mainted a written record of daily attendance that documents arrival and departutre of each child in care as it occurs.
Evidence:

Standard #: 8VAC20-780-430-K Description: Based on observation, the center did not ensure provision was made for an individual place for each child's personal belongings.
