Technical Assistance:
Today we discussed supervision and which staff are being counted in ratio.

Comments:
An unannounced monitoring inspection was initiated and concluded on May 24, 2022. There were five children present at the onset of inspections ages 2.5 years through 5 years old. There were three staff present in the building. At the conclusion of the inspection there were 21 children present with three staff. A sample of children and staff records were reviewed. A walking tour of the building was completed including two classrooms and restrooms. Annual inspections, insurance, first aid kit and emergency evacuation drills were reviewed. Children were observed in teacher and self-directed activities including snack, transitions, restroom routines and manipulatives. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice and reviewed with the center representative during the exit interview at the conclusion of the inspection.

Description: Based on record review the center failed to obtain required background checks within allotted time frames.
Evidence:
The record for staff 2 has a documented date of hire as March 21, 2022. However, there was not any documentation that a Virginia, Central Registry finding had been requested.

Description: Based on observation the center failed to ensure that areas are maintained in safe condition.
Evidence:
1 - In the orange classroom there is set of plastic drawers that are collapsing on itself. It appears the items in the drawers are too heavy for the plastic drawers they are stored in.
2 - There are dangling cords within reach of children in the large multipurpose room in two areas: - the round table at the main entrance to the room and - the wall/corner area of the multipurpose room to the right of the door way used by children to access the restrooms.
Dangling cords pose an entanglement and potential toppling hazard.

Description: Based on observation the center failed to ensure that cosmetics, medications, or other harmful agents are not be stored in areas, purses or pockets that are accessible to children.
Evidence:
1 - There was a staff purse sitting on an open shelf in the multipurpose room.
a. The shelf was an unlocked place location
b. The shelf was approximately 2.5 feet up from the floor and therefore accessible to all children in care.
2 - Staff 1 was observed as she retrieved the purse, withdrew an inhaler and used the albuterol device.

Description: Based on observation the center failed to ensure that children under the age of 10 years old are within actual sight and sound supervision unless children are in the restroom.
Evidence:
During the inspection while staff 1 was left alone to care for five children ages two and half years through five years old, she walked out of the classroom and into the lobby to answer the front door bell two times. This action left all five children unattended by any staff.

Evidence:

Description: Based on observation the center failed to ensure that provided snacks follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence:
1 - The children were in care were served oranges and water for snack.
a. The amount of oranges served was not a measured amount.
b. USDA requires components from two of the six food pyramid groups. Only one component was served.

Description: Based on observation the center failed to sanitize tables before and after each use for feeding.
Evidence:
After a staff wiped off the table used by children when eating snack, the staff reported that the dish towel being used contained soap and water. There was not any sanitizer used
