Comments:
A complaint investigation was initiated on May 20, 2022 with another local agency and ended on June 23, 2022 regarding a four year old child left unsupervised in the classroom. Interviews were conducted and documentation were reviewed. There was a preponderance of evidence to support the allegation regarding supervision. The complaint was deemed valid.

The complaint related violations and one non-related violation are identified in the Violation Notice.

Description: Based on center's policies and procedures, the center's own policies and procedures does not provide information how staff are supervising children to ensure their care, protection and guidance.
Evidence:
1. The policy does not address how supervision would be implemented during transitions.
2. There was no system in place to assure children who are using the restroom are provided supervision under the age 10 years.

Policies do not outline how staff supervision is maintained to ensure that the staff are supervising childen as required by these standards.

1. The center's polices and procedures does not provide detail on how supervision will be provided for transitions, seeing and hearing children, and how staff will directly be supervising children.

Plan of Correction: We have rewritten our supervision policy to better reflect supervision during transitions and when the children are using the restroom.

Complaint related: Yes
Description: Based on interviews and documentation, the center did not ensure staff were supervising children to ensure their care and protection.
1. In the four to five year old classroom, a child was heard crying in the bathroom.
2. A four year old child was left unsupervised in the classroom and unattended by staff.
3. There were three staff in the classroom on the day of the incident. Two staff members did a head count and counted 27 children. It was 28 children present in the classroom. Outdoor play is from 4:30 pm to 5:00 pm according to the daily schedule.
4. Two staff members did a head count. They did not do name to face recognition when counting the children during transition. The staff did a head count before they left the classroom and again when they entered on the playground.
5. Assistant Director heard child crying in the bathroom. Child was left unsupervised for about 10 minutes. The Assistant Director took the child to the playground once child was finished using the bathroom.

Plan of Correction: We have rewritten our supervision policy to better reflect supervision during transitions and when the children are using the restroom.
