Comments:
A monitoring inspection was conducted on 6/28/2022 with the center director and staff. There were 37 children present, ranging in ages from 5 months to 9 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 0 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-C
Description: Based on staff interview, the inspector was not able to inspect all aspects of the program during today's inspection. Evidence: 1. An unannounced inspection was conducted from 2:30 p.m. to 5:00 p.m. on June 28, 2022. The director stated that the owner was on vacation and the office that has employee files and other records was locked and no key was present. 2. The director stated that it is the policy to lock the office door when the owner is not on site.

Description: Based on staff interview, the facility failed to have staff files accessible which contain the name, address, and phone number of a person to be notified in case of an emergency. Evidence: the director stated that the staff files were locked in the office and she could not access the emergency contacts for staff if needed.

Description: Based on observation, the facility filed to maintain all areas in a clean manner. Evidence: in the infant room and the twos/threes room, the high chair seat covers, rugs, and carpet were visibly dirty with black marks, stains and dirt residue.

Description: Based on observation, the facility did not have children under the age of 10 years of age in actual sight and sound supervision at all times. Evidence: when the licensing inspector arrived at approximately 2:30 p.m., there were five school age children in the front room alone with no staff present. After a few minutes, the staff person came into the room from the back where she was getting food trays with afternoon snack/dinner.

Description: Based on observation, the facility failed to prevent the use of pillows and filled comforters for children under the age of two years. Evidence: In the infant room, there were four pillows and one filled comforter in four cribs. Evidence: Two infants, aged 5 months old and 8 months old, were sleeping in cribs, propped by boppy pillows. Two older infants had pillows in their cribs; one infant had a a filled comforter type blanket.

Description: Based on observation, the facility failed to have staff and children wash hands with soap and running water after toileting or diapering. Evidence: Staff changed a child?s diaper in the infant room and did not wash the child's hands with soap and water. Staff did not wash their own hands immediately before or immediately after changing the child?s diaper.

Description: Based on observation, the facility failed to follow diapering procedures as required. Evidence: the portable sink in the infant room was not working due a missing part. The diaper disposal system (trashcan) did not have a foot pedal accessible. Staff disposed of diapers in a lidded trash can that was not foot operated or used in such a way that the staff member's hand nor the soiled diaper touched an exterior surface of the storage system during disposal.
