Comments:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, June 9, 2022 from 11:20am to approximately 1:20pm. There were a total of 20 children in care in the direct care of five staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were reviewed and found to be in compliance. During the inspection, five children?s records and five staff records were reviewed. Additional information was later submitted to the inspector and was reviewed virtually.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on a review of five staff records and interviews, the center did not ensure that one staff member had a central registry finding within 30 days of employment.

Evidence: 1) The central registry finding in the record for Staff #2, hired on 09/27/21, was dated 01/31/22. 2) During interview, a member of management confirmed the central registry finding for Staff #2 was not obtained within 30 days of employment. The record did not contain documentation of any further contact and the staff had been continuously employed.

Plan of Correction: In the future, the center will document further contact with OBI when the central registry finding is not received within 30 days of employment.

Standard #: 8VAC20-780-160-C
Description: Based on a review of five staff records and interviews, the center did not obtain the results of a follow-up tuberculosis (TB) screening for two staff members at least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department

Evidence: 1) The most recent TB screening/testing in the record for Staff #4, hired on 06/01/17, expired on 03/02/22. 2) The most recent TB screening/testing in the record for Staff #5, hired on 03/01/16, expired on 02/20/22.

Plan of Correction: Staff will obtain new TB screenings as soon as possible.

Standard #: 8VAC20-780-70
Description: Based on a review of five staff records and interviews, the center did not ensure that one staff record contained all the required information.

Evidence: 1) One of the two references in the record for Staff #2, hired on 09/27/21, was obtained on 10/08/21. 2) The record for Staff #3, hired on 04/21/22, was missing a second reference. 2) During interview, a member of management acknowledged the reference was not obtained before employment. Each staff record should contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Plan of Correction: In the future, all references will be obtained prior to employment.
