Comments:
An unannounced complaint inspection was conducted on 5/17/2022 to investigate an allegation received by the department related to Supervision of Children and Nutrition and Food Services. Based on evidence gathered through interviews and documentation review, the portion of the complaint related to Supervision of Children was determined to be valid. The portion of the complaint related to Nutrition and Food Services was not valid. A violation notice was issued to the facility. An exit interview was conducted on 7/7/2022.

Description: Based on review of documentation, not all staff had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for staff #1 (date of hire: 8/28/17) had a TB test/screening dated 2/23/20.

Description: Based on review of documentation, and multiple interviews, the center has agreed to administer medication but did not always have a staff member on site with current Medication Administration Training (MAT) certification. Evidence: Staff #1 was working alone from approximately October 12, 2021 to April 21, 2022 between the hours of approximately 6:00 ? 7:30 am. Staff #1 does not have documentation of MAT certification. Interviews revealed that there was medication on site during this time period.

Description: Based on observation, review of documentation, and multiple interviews, the center failed to ensure that a child for whom emergency medication has been prescribed was always in the care of a staff member who has current Medication Administration Training (MAT) certification. Evidence: Child #1, with a diagnosed food allergy, and medication (epinephrine) on site, was in attendance April 11, 12, 13, 14, 19 and 20, 2022 from approximately 6:30 ? 7:25 am. Staff #1 was working alone during this time period and does not have MAT certification.

Description: Based on review of documentation, and multiple interviews, the center failed to have a second staff member or volunteer on the premises when one or more children are present. Evidence: from approximately October 12, 2021 until April 21, 2022 from approximately 6:00 am ? 7:30 am, only one staff member was regularly scheduled to work. Interviews revealed that there was only one staff member working during this time period. There were approximately 6-9 children in attendance during this time period.

There were approximately 6-9 children present during this time period.
