A monitoring inspection was conducted on 07/07/2022. There were (#) children present and (#) staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 32 children?s records and 10 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.058 Description: Based on observation the center did not have at least one carbon monoxide detector.
Evidence:
The program director was not able to locate the portable carbon monoxide detector the landlord had provided.Plan of Correction: The owner will contact the church and have a carbon monoxide detector installed as soon as possible.
Standard #: 22VAC40-185-260-A Description: Based on a record review the center did not have documentation of an annual fire inspection.
Evidence:
The last fire inspection of the center was dated 03/16/2021Plan of Correction: The director will call the fire marshal today and ask to schedule an inspection. The phone call will be documented.
