Physician's signatures on immunization records were discussed.
- Comments:

A monitoring/renewal/complaint inspection was conducted on 7/7/2022 . There were 30 children present, ranging in ages from 2 yrs to school age, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 children's records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Standard #: 8VAC20-780-270-A Description: Based upon observation and staff interview, the facility has not ensured that areas and equipment outside of the facility are maintained to be safe.
Evidence:
There I are several areas on the toddler playground where poison ivy vines have grown through the fencing.
Staff 1 acknowledged that the poison ivy vines are within reach of children on the playground.Plan of Correction: The facility responded with the following:
All poison ivy will be removed from within reach of the children. Routine inspections will be conducted o ensure that no poison ivy or other vines are growing into the playground areas.
