Comments:
The licensing inspectors conducted an unannounced complaint inspection in response to a complaint received by the department on 03/29/22 regarding the center?s physical plant and building maintenance. The investigation was initiated on 04/28/22 and concluded on 07/12/22. On 04/28/22, the inspectors were on site from 9:50am to approximately 10:35am. Interviews were completed, observations were made at the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports part of the allegations, therefore the complaint is determined to be valid. Any violations not related to the complaint, but identified during the course of the investigation can be found on the violation notice.

Description: Based on observations and interview, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.

Evidence: During the inspection on 04/28/22, the following was observed:
1) An electrical outlet in an upstairs room was observed with no face plate covering, exposing wires and peeling paint. This was accessible to the children.
2) A doorknob in the June Bugs classroom was lose, creating a pinching hazard.
3) One of the picnic tables located on the preschool playground was damaged with deteriorating wood and a crack in the middle of the table top. It was reported the children use the table.
4) Two PVC pipes were observed protruding out of the ground on the side of the playground where the children play, creating a tripping hazard. It was reported two tables were removed and the umbrella stands were exposed.

Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Plan of Correction: The center provided photographic evidence of the corrections made. The doorknob was tightened, the face place covering was replaced, and the areas with peeling paint were repainted. Two umbrellas to cover the PVC pipes were ordered and the picnic table was replaced.
