Technical Assistance:
Discussed sunscreens should be labeled with the child's name. Discussed the allergy list and the need to keep it confidential. Discussed the medication forms that need doctor's signatures in order for them to be used form more than 10 days.

Comments:
An unannounced monitoring inspection was conducted on 7/21/2022 from 9:23am to 12:40pm. At the time of entrance,37 children were in care with 9 staff members present. Children were observed doing the following: listening to a book the teacher was reading, playing with toys, sleeping, singing and playing with musical instruments, diaper changes, washing hands, Montessori activities, and playing outside. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, first aid supplies, medications, evacuation drills and attendance records were reviewed. Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the infant classroom, bleach water was in a basket near the changing table.
2. In the Senior Toddler classroom, bleach water was in a basket near the changing table.
3. In the School age classroom, bleach water, betta fix conditioner and stress zyme were in a basket near the sink.

Plan of Correction: Lock bleach water bottles in the closet
in each classroom

Description: Based on observation, the center had a current written list of all children's allergies, sensitivities, and dietary restrictions, but did not keep it confidential in each room or area where children were present.
Evidence:
1. In the preschool classroom, the allergy list was posted on a board and the information was not kept confidential.

Plan of Correction: Added a cover

Description: Based on observation, electrical outlets did not all have protective covers and serve children preschool age or younger.
Evidence:
1. In the infant classroom an outlet near the rocking chair was not being used and not covered with a protective cover.

Plan of Correction: Re-train teachers to ensure that unused
outlets are always covered

Description: Based on observation, the center did not meet the diapering requirement of having a nonabsorbent surface for diapering.
Evidence:
1. In the infant changing table had 9 rips in the diaper changing pad and was not nonabsorbent.

Description: Based on observation, medication was not labeled with the child's name, name of the medication, the dosage amount and the time or times to be given.
Evidence:
1. Child A had an emergency medication in a bag that did not have a prescription label, dosage amount and the time or times to be given.

Plan of Correction: Contaced parents immediately.

Description: Based on review of medications and interview, the center did not dispose of medications when an authorization for medication expired and the parent did not pick it up or renew the authorization within 14 days.
Evidence:
1. Child B had an authorization for a medication that expired on 7/01/2022. Staff A stated they had the medication and had not been returned or disposed of.

Plan of Correction: Contacted the parent immediately for
an updated authorization form

Description: Based on observation, the center did not meet all diaper ointment or cream requirements.
Evidence:
1. In the infant classroom, 6 diaper ointments and creams were located in a lower shelf within reach of children.

Description: Based on observation, the center did not follow all requirements when food is brought from home.
Evidence:
1. In the infant classroom, a small container of brown rice was in a child's cubby box, but it was not clearly dated and labeled in a way that identifies the owner.

Description: Based on observation, prepared infant formula was not dated.
Evidence:
1. 4 infant formula bottles were labeled with the child's name, but did not have a date on them.
