Comments:

An unannounced monitoring inspection was conducted on July 14, 2022 from approximately 10:00 am to 11:50 am. There were 64 children in attendance and a total of 16 staff present. 6 staff records and 6 children?s records were reviewed. All areas of the center were observed including classrooms, hallways, kitchen, and bathrooms. The children were observed playing with table toys, creating ocean animals, having circle time, playing word games, listening to music, playing outside, and getting diapers changed, while the licensing inspector was on the premises. Emergency drills and inspections were in compliance. The most recent Health Inspection was on 07/11/22; Fire Drills are conducted monthly with the most recent on 06/15/22. Shelter in Place Drills were conducted twice with the most recent on 03/31/2022. The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted by each phone. The menu was posted. Lunch was being prepared and consisted of chicken nuggets, peas, peaches, and milk. Violations were cited as a result of this inspection. Standard #:	8VAC20-780-130-A
Description:	Based on a review of documentation, the center did not ensure to obtain immunization records for each child on or before the first day of attendance for 1 out of 6 children?s files reviewed. Evidence: Child #5?s record (age 3 years), enrollment date 11/26/19, did not contain evidence of immunization records upon enrollment or any updated immunizations.
Plan of Correction:	We have developed a new plan for incoming families, complete with a file checklist. With this, we will begin setting internal calendar invites for a week before the start date, the day of, and a week after the start date to follow up should any additional information be required.
Standard #:	8VAC20-780-140-A
Description:	Based on a review of documentation, the center did not ensure to obtain a physical for each child within 30 days after the first day of attendance for 1 out of 6 children?s files reviewed. Evidence: Child #5?s record (age 3yrs) date of enrollment 11/26/19, did not contain evidence of an original physical exam or any updated physical exams.
Plan of Correction:	We have developed a new plan for incoming families, complete with a file checklist. Description:	Based on a review of documentation, and interview, the center did not provide to the Licensing Inspector an annual fire inspection report. Evidence: The last fire inspection was dated 03/09/2021. The Director stated that he had tried to call to schedule it but they had not come out yet.
