Technical Assistance:
Reviewed standards regarding car seats during transportation and playground equipment.

Comments:
An unannounced monitoring inspection was conducted on August 17, 2022, from 8:45am to 10:50am. There were 47 children present and 9 staff members. The Inspector reviewed five children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during breakfast, group activity, outdoor play, diapering, and individual reading. First aid kits, emergency preparedness supplies/documents, and transportation vans were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the owner at the exit interview.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing staff members obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1. Two of the five staff files reviewed have tuberculosis screenings that are past two years from the date of the initial screening/testing.
a. Staff #1 , Director, has a tuberculosis test dated 8/11/2020.
b. Staff #2 has a tuberculosis test last dated 8/11/2020 which is outside of the required timeframe.
2. Staff #1 confirmed the dates are past the required timeframe of two years.

Plan of Correction: The center responded with the following: Staff #1 will schedule an appointment today for both tuberculosis screenings to be completed by the end of the week. The results of a negative test will be placed in the staff record.

Standard #: 8VAC20-780-70
Description: Based on observation and interview, it was determined that the center did not ensure that staff records be kept for each staff person to include a name, address, and telephone number of a person to be notified in an emergency which shall be kept at the center.
Evidence:
1. Staff #3 (date of hire 8/9/22) did not have the address of a person to be notified in an emergency.
2. Staff #1 confirmed there was no address listed to the emergency contact for Staff #3.

Based on observation and interview it was determined that the center did not ensure that staff records be kept for each staff person to include documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.
Evidence:
1. Staff #3 (date of hire 8/9/22) did not have two references checks completed to his record.
2. Staff #1 confirmed that Staff #3 did not have reference checks completed prior to employment.

Plan of Correction: The center responded with the following: Staff #1 will obtain the address for the emergency contact for Staff #3.

The center responded with the following: Moving forward, the center will ensure that all reference checks are completed prior to employment and documented to their record.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interview, it was determined that the center did not ensure that when food is brought from home the food container be clearly dated and labeled in a way that identifies the owner.
Evidence:
1. In the preschool room, there were three water bottles not labeled in a way that identifies the owner.
2. Staff #1 confirmed that the water bottles were not labeled.

Plan of Correction: The center responded with the following: All water bottles will be labeled.
