Technical Assistance:
Technical assistance was provided in the following areas of the standards: Background checks, play areas, children's records, staff records, Program leader qualifications, hand washing, and nutrition.

Comments:
An unannounced monitoring inspection was conducted on 8/23/22 from 12:45pm - 2:45pm. During the inspection there were 35 children ages five years old through twelve years old in care with five staff. Children were observed participating in various activities in the playing golf and eating lunch. Records were reviewed for five children and ten staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit
interview.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on a review of six staff records, it was determined that the licensee did not ensure that the facility did not ensure that an employee must not be employed until a sworn statement or affirmation has been completed.

Evidence:
1. The record for staff #1, working during the inspection, did not contain a sworn statement
or affirmation.
2. Staff #6 (Program Director) reviewed the record for staff #1, and confirmed the sworn statement or affirmation was not completed prior to employment

Plan of Correction: The facility responded: Staff #1 has completed a sworn statement. All new employees will not begin employment unless a sworn statement is completed.

Standard #: 8VAC20-770-60-C-2
Description: Based on a review of six staff records, it was determined that the licensee did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of

Evidence:
1. The record for staff #2 (date of hire 6/28/21) did not contain documentation of a completed
search of the central registry finding.
2. The record for staff #3 (date of hire 6/16/22) did not contain documentation of a completed search of the central registry finding.
3. The record for staff #4 (date of hire 6/20/22) did not contain documentation of a completed
search of the central registry finding.
4. The record for staff #5 (date of hire 6/22/21) did not contain documentation of a completed
search of the central registry finding.
5. Staff #6 (Program Director) reviewed the records for the staff listed above, and confirmed that the search of the central registry finding has not been received within 30 days of employment.

Plan of Correction: The facility responded: These staff have previously submitted a search of the central registry. If the results can't be found, they will complete a new search of the central
registry.

Standard #: 8VAC20-780-160-A
Description: Based on a review of six staff records, it was determined that the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and shall
have been completed in the last 30 calendar days.

Evidence:
1. The record for staff #4, working during the inspection did not contain documentation of a
negative tuberculosis screening.
2. Staff #6 (Program Director) reviewed the record for the staff #4, and confirmed that the documentation of a negative tuberculosis screening had not been received.

Plan of Correction: The facility responded: Staff #4 will be sent to complete a TB screening. All new staff will
complete a TB screening prior to beginning employment.

Standard #: 8VAC20-780-340-D
Description: Based on observation, a review of six staff records and interviews, it was determined that the licensee did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Evidence:
1. The Licensing Inspector observed staff #1, staff #3, and staff #5 each with a separate group of children on the golf course.
2. None of the records for staff present with the children contained documentation to demonstrate any of the staff were Program Leader qualified.
2. Staff #6 (Program Director) confirmed that there was no documentation available for viewing during the inspection to demonstrate that any of the staff were Program Leader qualified.

Plan of Correction: The facility responded: We will ensure that there is a Program leader qualified staff in each grouping of children.

Standard #: 8VAC20-780-530-A
Description: Based on a review of six staff records and interview, it was determined that the licensee did not ensure that there is at least one staff in each classroom or area where children are present.

Evidence:
1.Staff #1, staff #3, and staff #5 each were working alone with a group of children, however none of their records contained documentation of a current CPR/first aid certification.
2. Staff #6 (program Director) confirmed there was not a staff present in each grouping of children with a current certification in CVPR/first aid.

Plan of Correction: The facility responded: All staff will complete the current CPR/First Aid certification. We
will ensure that there is always one staff present in each grouping of children with a current CPR/First Aid certification.

Standard #: 8VAC20-780-530-C
Description: Based on a review of six staff records and interviews, it was determined that the licensee did not ensure that there shall be at least two staff members who meet the requirements of subsection A of this section present on the premises during the center's hours of operation, on fieldtrips, and wherever children are in care.

Evidence:
1. The Licensing Inspector reviewed six staff records and only staff#6 had a current CPR/First aid certification.
2. Staff #6 (Program Director) confirmed that there was not two staff present with a current CPR/First aid certification.

Plan of Correction: The facility responded: All staff will complete the current CPR/First Aid certification. We
will ensure that there is always two staff present with a current CPR/First Aid certification.
