Technical Assistance:
Technical assistance was provided in the following areas:
1. 510-P: When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
2. 40-M: The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

Comments:
An unannounced monitoring inspection was conducted today from 11:30am-1:15pm. There were 36 children directly supervised by 9 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 4 staff records, 5 children?s records, 9 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed listening to story time, transitioning to nap time and napping. Handwashing and diapering procedures were observed and found to be in compliance. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center did not ensure that prepared infant formula is dated.

Evidence: 2 of 5 bottles of prepared infant formula observed in the refrigerator in the Baby Steps 1 room were not dated.

Plan of Correction: Bottle handling policy and procedures were gone over with all infant room staff in training.
