Comments:
A complaint inspection was initiated in response to a complaint received by the department on 03/10/2022 regarding allegations of behavioral guidance and the physical health of staff. The investigation was initiated on 03/22/2022 and completed on 08/15/2022. On 03/22/2022, the inspector was on site from 8:30am to approximately 10:15am. Interviews were completed, observations were made at the center, and documentation was reviewed. The inspector completed additional interviews and follow-up activities at the facility on 04/05/2022 from approximately 10:30am to 1:45pm. Additional interviews were conducted off-site and additional information was submitted to the inspector electronically. The preponderance of the evidence gathered during the investigation supports part of the allegations, therefore the complaint is determined to be valid.

Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

The provider's responses for the 'plan of correction' were not received as of 08/25/2022 and will not appear on this Violation Notice.

Description: Based on interviews, the center did not ensure that in order to promote the child's physical, intellectual, emotional and social well-being and growth, staff interact with the child to provide needed help, comfort and support.

Evidence: 1.Staff interviews revealed that Staff #1 responds to the children in a negative manner. Staff stated Staff #1 uses a "harsh" tone of voice and is "demeaning", "mean" and "rude" to the children.

Plan of Correction: The employee was terminated. The current Director will provide coaching and observations for employees to ensure help, comfort, and support are provided for all children in our center. The current Director will document observations and coaching provided to all employees on a continuing basis.
