Standard #: 8VAC20-780-340-A Description: Based on review of documentation and interview, the center did not ensure that when staff are supervising children, they always ensure their care and protection, and guidance.
Evidence:
1. On 8/22/2022, teacher #1 (date of employment: 01/28/2022) left the infant/toddler classroom for approximately one (1) to two (2) minutes, leaving three (3) children, ranging in age from seven (7) months to 18 months unsupervised. Teacher #1 stated that she heard crying and when she returned, another child was on top of child #1. Child #1 (age 12 months) sustained multiple bite marks and abrasions to the face and lower arms. Child #1 received medical care to treat the injuries on 8/22/2022.

Standard #: 8VAC20-780-340-G Description: Based on review of documentation and interview, the center did not ensure that children under 10 years of age were always within actual sight and sound supervision of staff.
Evidence:
1. In interview, teacher #1 (date of employment: 01/28/2022) stated that she had four (4) children in care and one (1) child needed a diaper change. She took the one (1) child to the classroom next door and changed his diaper. Staff #1 stated that she ?understood that the children were not in her direct sight? when she left the room to do a diaper change.
