The department received a complaint incident regarding allegations in the areas physical plant, staffing and supervision on March 10, 2022. A complaint inspection was initiated on March 17, 2022 and concluded on April 14, 2022. The licensing inspector was onsite on April 14, 2022 from approximately 8:45am until 9:45am. Observations were made at the center, interviews with staff were completed and documentation was reviewed.
The evidence gathered during the investigation did not support the allegation of non-compliance with standards or law. Any other violations not related to the complaint but identified during the course of the investigation can also be found on the violation notice.

Standard #: 22.1-289.035-A Complaint related: No Description: Based on review of two staff records, the center did not ensure all required background checks were repeated at least every 5 years.
Evidence:
1. The record for Staff #1 (hire date 08/19/1990) contained a central registry finding dated 10/31/2016.
2. A member of management confirmed a repeat central registry finding was not on file for Staff #1.Plan of Correction: We have requested the central registry update and will add it to the record.
