Comments:
An unannounced monitoring inspection was conducted on September 13, 2022 from approximately 10:15 am to 11:40 am. There were 28 children in attendance and a total of 7 staff present. 4 staff records and 4 children?s records were reviewed. All areas of the center were observed including classrooms, bathrooms, hallway, kitchen, and the playground. The children were observed listening to music, participating in circle time, painting green turtles, and playing outside on the playground while the licensing inspector was on the premises.

The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted by each phone. The most recent health inspection was on 1/12/22. The most recent fire inspection was on 10/4/21. Fire drills were conducted monthly with the most recent on 9/6/22. Shelter in place drills were conducted twice with the most recent on 9/2/22.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on interview, the center failed to maintain a current written dated list of all children?s allergies in each room or area where children are present. Evidence: When asked, the teachers and Director stated that they did not have a written allergy list.

Plan of Correction: Per the Director "list of allergies are posted in all rooms."

Standard #: 8VAC20-780-60-A
Description: Based on records review, and interview, the center failed to obtain a written care plan for each child with a diagnosed food allergy, to include instruction from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for child #1, date of enrollment 08/13/2022, and the record for child #3, date of enrollment 08/18/21, each had documentation by the parent that the child had an allergy to nuts. Both children have an epi-pen on site at the center. Neither record contained a written allergy care plan. When asked, the Director stated she did not realize she needed to have that.

Plan of Correction: Per the Director "written allergy plan is in double locked bags. These plans were give to the secretary in the office."
