Description: Based on review of staff records, the center failed to ensure that the Virginia Department of Education sponsored orientation course shall be completed within 90 calendar days of employment.

Evidence:
Staff #1's record does not have documentation of the completion of the Virginia Department of Education sponsored orientation course.

Plan of Correction: The Director will inform staff member of course requirement and ensure that course is completed and documented.

Description: Based on observation, the center failed to ensure that areas and equipment of the center will be maintained in a clean, safe and operable condition.

Evidence:
1. In Infant 1 and Infant 2 classrooms the fabric floor mats are cracked and frayed.
2. In the Sr. Preschool classroom, the paint is peeling from the wall.
3. In the Infant 2 classroom, the temperature on the thermometer had a reading of 54 degrees.

Plan of Correction: I will contact the owner regarding the frayed mats, peeling paint and temperature in the Infant 2 classroom refrigerator.

Evidence:
There were two staff purses in classrooms Jr. PK1 and Jr. PK2, that were accessible to the children.

Plan of Correction: Purses were put in areas not accessible to children during the inspection.

Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Evidence:
In the Sr. Preschool classroom, an outlet cover was missing near the shelves on the back wall.

Plan of Correction: The Director will replace the outlet cover.

Description: Based on observation, the center failed to ensure that medication shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the Toddler 1 classroom, medication was not kept in a locked place using a safe locking method.

Plan of Correction: Medication was placed in an operable Safety bag, locked and returned to the classroom.

Description: Based on observation, the center failed to ensure that all required items were included in the first aid kit.

Evidence:
In Toddler 1 classroom, the first aid kit was missing the following required items - gauze pads, antiseptic cleaner, gloves, a thermometer and an ice pack.

Plan of Correction: The Director will replace required items for the First Aid Kit.

Description: Based on observation the center failed to ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:
In the Infant 2 refrigerator, the child's food had the child's name, but was not dated.

Plan of Correction: The staff will remind parents to label and date all food brought from home. If parents failed to label and date food, the staff will apply labels and dates.
