An unannounced monitoring inspection was conducted on 9/14/2022. There were 20 children, ages 5 -11 years and 3 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 2 staff records were reviewed. The children were observed during arrival, snack and activity center time. The inspector arrived for the inspection at 3:47pm and departed the center at 4:50pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22.1-289.035-B-2 Description: Based on review of three staff?s records, the center failed to ensure that all records were complete.
Evidence: Staff #1 was hired on 8/23/2022 and on the day of the inspection did not have the required criminal history record check prior to the first day of employment.Plan of Correction: The provider will submit a criminal history check for staff.
Standard #: 8VAC20-780-70 Description: Based on review of three staff?s records, the center failed to ensure that all records were complete.
Evidence:
1. Staff #1 and Staff #2 were hired on 8/23/2022 and on the day of the inspection did not have documentation to demonstrate education, certification or experience required.
2. Staff #1 was hired on 8/23/2022 and on the day of the inspection did not have two references documented.
3. Staff #1 was hired on 8/23/2022 and on the day of the inspection did not have an emergency contact documented.Plan of Correction: The provider will obtain missing information in staff records.
Standard #: 8VAC20-780-260-A Description: Based on observation, the center failed to meet requirements subsequent to initial licensure.
Evidence: The fire inspection posted was dated 5/10/2019 and on the day of the inspection did not have the most recent fire inspection posted.Plan of Correction: Provider will obtain current the fire inspection.
Standard #: 8VAC20-780-260-B Description: Based on observation, the center failed to meet requirements subsequent to initial licensure.
