Comments:
An unannounced monitoring inspection was conducted on 09/15/2022 from 9:05 a.m. to 12:40 p.m. There were 38 children present, ranging in ages from 8 months to 4 years old, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 5 child records and 6 staff records were reviewed
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, the center failed to ensure that the center?s equipment was maintained in clean, safe and operable condition.
Evidence:
1. On the playground, 2 riding toys (1 riding car and 1 trike) were observed to have rust.
Staff #7 confirmed that the riding toys were rusted.

Plan of Correction: The rusted equipment has been removed from the playground. Staff has been instructed to throw away any toys that are broken or otherwise unsafe as soon as they see them. The playground will be periodically inspected by staff to ensure that any broken or dirty toys are disposed of.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, the center failed to ensure that all hazardous substances were kept in a locked place.
Evidence:
1. In Classroom #1, observed on a shelf next to the changing table were 2 containers of Great Value sanitizing wipes, 4 bottles of commercial antibacterial spray and one bottle of Lysol bathroom cleaner.
2. Staff #2 confirmed that the hazardous materials were not kept in a locked place.

Plan of Correction: Staff has placed the hazardous materials in a locked closet, and will ensure that they are locked away after use. Periodic walkthroughs will be conducted by staff to ensure that hazardous materials are stored in the locked location.

Standard #: 8VAC20-780-320-B
Description: Based on observation, the center failed to ensure that the water temperature did not exceed 120F.
Evidence:
1. The water temperature in the sink in Classroom #2 was 130.5 degrees Fahrenheit, as measured by a thermometer.

Plan of Correction: Maintenance has adjusted the water temperature for the classroom faucets to reach a maximum of 120 degrees Fahrenheit.
