The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 9/20/2022 relating to the menu and not being informed of substituted food and that a teacher was yelling at children in the 2 year old classroom. Interviews were conducted with staff, a document summarizing a meeting with staff was reviewed, daily meals checklist from the catering services and menu were reviewed.
The information gathered during this investigation does support the allegations, so the complaint is determined to be valid. See the Violation Notice for findings.

Standard #: 8VAC20-780-400-A Complaint related: No Description: Based on the interview, staff members did not interact with one another to provide needed help, comfort and support in order to promote the child's physical, intellectual, emotional, and social well-being and growth.
Evidence:
1.Staff # 2 and Staff #4 work together in the Sr. Toddler class. Staff #2 stated ?[Staff #4?s] tone scared me and the kids at times?; and that Staff #4 ?verbally started yelling at me in front of the kids because I had reported her for not following the curriculum.?
2.In a document summarizing meetings with Staff #2 and Staff #4, Staff #1 wrote, ?In the morning of Thursday 9/15/2022 both [Staff #4 and Staff #2] were arguing in the classroom, loudly, which led to Staff #1 stepped in the classroom to understand what was happening [sic].?Plan of Correction: Both staff #2 and #4 were met with disciplinary action before the complaint
which led staff 4's subsequent resignation as she refused to sign off the warning letter.
Standard #: 8VAC20-780-400-B Complaint related: No Description: Based on interviews, the center did not ensure behavioral guidance was constructive in nature, age and stage appropriate, and intended to redirect children to appropriate behavior and resolve conflicts.
Evidence:
1.Staff # 2 and Staff #4 work together in the Sr. Toddler class. Staff #2 stated Staff #4 would ask children ?why are you like this?? ?I heard [Staff #4] yell at the children several times.? Staff #2 stated Staff #4 ?speaks very loudly.? Staff #2 also stated ?Staff #4 would drag the children by the arm to time out.? Staff #2 stated ?Staff #4 was aggressive and would grab the children by the arm.?
2.Staff #1 stated while in the hallway, she could hear Staff #4 yelling at a new child that was crying and spoke with her about it 2-3 months ago.Plan of Correction: 1. Every staff member goes through "Prohibited Practices" policy and signs off during the orientation 2. Whenever admins are made of any practice against the policy, the staff member would be sat down to go through re-training, being written up or being terminated depending on the severity.
Standard #: 8VAC20-780-560-F Complaint related: No Description: Based on discussion with a staff member, review of the daily meals checklist from the catering services and menu, and observation, the center failed to ensure that the menu listing foods to be served for meals and snacks during the current one week period listed any substituted food.
Evidence:
1.Staff #1 admitted the center changed the menu on 9/06/2022 and 9/07/2022 and did not notify the parents. Staff #1 stated they usually email the parents of any changes, but did not for those two days.
2.The Daily Meal checklist from the catering company for 9/6/2022 lists chicken nuggets. The center?s menu listed Cheese pizza for 9/6/2022. For 9/7/2022, the center menu listed Fish Filet, but the children were served cheese pizza.Plan of Correction: Corrected. Onsite administrator will remember to always email parents for any menu changes.
Standard #: 8VAC20-820-120-E-2 Complaint related: No Description: Based on observation, the findings of the most recent inspection of the facility was not posted.

1. The center did not have the findings of the inspection dated 9/14/2022 posted on the bulletin board during the inspection.Plan of Correction: Posted.
