Comments:
An unannounced monitoring inspection was conducted on September 28, 2022 from approximately 10:00 am to 11:00 am. There were 16 children in attendance and a total of 4 staff present. 2 staff records and 4 children?s records were reviewed. All areas of the center were observed including classroom, bathrooms, hallways, and the playground. The children were observed playing with table toys, eating snack, and listening to stories, while the licensing inspector was on the premises.

The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted. Fire drills were conducted monthly with the most recent on 8/1/22.

Violations were cited as a result of this inspection.

Description: Based on interview, and observation, the center failed to ensure that the center contains at least one carbon monoxide detector. Evidence: A carbon monoxide detector was not observed during the tour of the center. When asked, the Director was unable to confirm that the center was equipped with a detector.

Description: Based on records review, the center failed to obtain documentation that each child received immunizations as required before attendance at the center. Evidence: The record for child #1 (date of enrollment 09/6/22), and child #3 (date of enrollment 09/6/2022) did not contain documentation of immunizations

Description: Based on records review and interview the center failed to ensure that the terms of the current license were followed. Evidence: When interviewed, staff #1 stated that child #1 had just turned 2. Records review determined that child #1, date of enrollment 9/6/22, was 2 years 2 months at the time of enrollment. The current license is for the ages of 2 years 6 months to 11 years 11 months. When interviewed the Director stated she thought the license began at the age of 2 years old.

Description: Based on records review, the center failed to document viewing proof of the child?s identity and age in one out of four records. Evidence: The record for child #2 (date of enrollment 09/16/2021) did not contain documentation of viewing proof of the child?s identity and age.

Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The fence on the playground had several protruding nails and cracked, splintered, and deteriorating wooden slats.
