Comments:
An unannounced monitoring inspection was conducted on 9/22/22 from 5:05pm to 5:50pm with program staff. There were 16 children in care, supervised by three staff. The children were observed having a snack of cucumbers, dip, and wheat crackers and then transitioning to the playground to collect leaves for a project. Four child records and three staff records were reviewed. The Center has three staff with current certification in CPR and First Aid as well as trained in Daily Health Observation Training. Two medications and authorization forms were reviewed and the center has two staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The first aid kit, flashlight, and battery operated radio were observed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on October 3, 2022. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of three staff records, the center did not obtain documentation of the results of a search of the central registry for one staff within 30 days of employment. Evidence: The central registry check on record for Staff C, with a start date of 5/25/22, was dated 8/28/22.

Plan of Correction: Central Registry searches will be completed within thirty days of the employee's hire date. This will be effective immediately for all new employees.

Standard #: 8VAC20-780-160-A
Description: Based on review of three staff records, the center did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: 1. The TB screening on record for Staff A, with a start date of 1/31/22, was dated 3/28/22. 2. The TB screening on record for Staff B, with a start date of 4/7/22, was dated 5/18/22

Plan of Correction: TB screenings/tests will be completed prior to the employee's hire date. This will be effective immediately for all new employees.

Standard #: 8VAC20-780-240-A
Description: Based on review of three staff records, the center did not ensure that the Virginia Department of Education-sponsored orientation course was completed within 90 calendar days of employment for one staff. Evidence: The certificate on record for Staff C, with a start date of 5/25/22, was dated 9/25/22.

Plan of Correction: New staff orientation will be completed within 90 days of the hire date. This will be effective immediately for all new employees.
