An unannounced monitoring inspection was conducted today. There were 15 children present with two staff.
The children were eating lunch and playing outdoors.
Two child records and two staff records were reviewed.

Standard #: 22.1-289.035-A Description: Based on the review of two staff files, the facility failed to ensure that all staff have completed a repeat background check every five years.
Evidence:
1. There is no repeat central registry search on file for staff #2. The clearance on file is dated 4/27/17.Plan of Correction: The program manager will ensure that the repeat central registry clearance is completed.
