A monitoring inspection was initiated on 9/21/2022 and concluded on 9/21/2022. There were a total of 34 children with 7 staff members present. Children were observed engaged in station activities, playing with toys, having individual assistance with letters, and preparing for snack time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-160-C Description: Based on file review, a staff member did not submit the results of a follow-up tuberculosis screening at least two years from the date of the first initial screening or testing.
Evidence: Staff #1 did not submit the results of a follow-up tuberculosis screening at least two years from their previous screening that occurred 9/10/2020.Plan of Correction: Center will ensure that staff member's tuberculosis screening is taken care of and documentation is filled.
Standard #: 8VAC20-780-270-A Description: Based on observation, areas of the center were not maintained in a clean, safe, and operable condition.
Evidence: There was chipped paint and plaster near the window in the 3's classroom.Plan of Correction: Center will have the chipped paint/plaster repaired.
Standard #: 8VAC20-780-280-B Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place that prevents access by children.
Evidence: Cleaning wipes were found in an unlocked cabinet in the 4's classroom.Plan of Correction: Center moved the cleaning wipes to a locked location.
Standard #: 8VAC20-780-510-B Description: Based on a review of stored medication and children's file review, the center was not able to follow the written parental authorization instructing that non-prescription medication be given to a child .
Evidence: The non-prescription medication for Child #1 was not available at the center.Plan of Correction: Center will contact parents of Child #1 to obtain the needed non-prescription medication.
