An investigation was initiated from 1:15 pm - 3:00 pm to investigate a complaint regarding reporting, supervision, and care, protection, and guidance. Interviews were conducted, staff files, child files, and documentation was reviewed. The complaint regarding reporting, supervision, and care, protection, and guidance is determined to be valid. Standard #: 8VAC20-780-80-B Complaint related: Yes Description: Based on interviews it was determined that the facility failed to report all incidents to the department when a child is missing and local authorities are contacted for help. Evidence: On 01/06/2022 at 1:35 pm child A (d.o.b. 07/25/2018) was found standing on the side of Zachary Taylor Highway by a citizen driving by the facility. The citizen called local authorities who were dispatched to the facility. Administration for the facility failed to report this incident to the department. Plan of Correction: Not available online. Standard #: 8VAC20-780-340-A Complaint related: Yes Description: Based on interviews with staff it was determined that staff did not always ensure the care, protection, and guidance of children under their supervision. Evidence: On 01/06/22 at 1:35 pm, child A (d.o.b. 07/25/2018) was participating in a sledding activity outdoors with their class. When it was time to go inside, children were asked to line-up and walk towards their classroom. While walking to the classroom child A left the group. When the group of children entered the classroom a headcount was conducted and staff realized that a child was missing. Staff A walked back outside to look for the child around the campus. During the time that staff were searching for the missing child, child A made their way to the side of Zachary Taylor Highway. While child A was standing on the side of the road a passerby pulled over to assist the child. The citizen then called emergency authorities and waited with child A until authorities arrived. Staff at the facility found child A once emergency authorities arrived at the school. Based on documentation it appeared to be a ten minute timeframe that child A was missing from their assigned group. Child A was reported to be crying when found by authorities. Evidence: On 01/06/22 at 1:35 pm, child A (d.o.b. 07/25/2018) got in line to go back to their classroom from an outdoor activity. While walking to the classroom child A left the group. When the group of children entered the classroom a headcount was conducted and staff realized that a child was missing. Staff A walked back outside to look for the missing child around the campus.
